SAMPLE:
24 year old female. Chief complaint of chills and slight fever, starting three days ago. Patient reports strong aversion to cold, slight fever, chills predominant, no perspiration, headache, aching joints and limbs, runny nose, scratchy throat, cough with thin white phlegm, without sensations of excessive thirst.
Tongue: Thin white coat, slight moisture
Pulse: floating and tight

Out of a in-depth intake, this is the crucial information for the pattern that we see forming. When taking exams involving case studies, you are constantly being tricked by information you do not need. Look at the questions that are ask which relate to the case. If the questions do not relate to past history, then that information is irrelevant to the case and usually does not apply. Generally focus on what is going on now, the other information is usually there to confuse your thought process. In addition, the tongue and pulse generally play important roles in determining the diagnosis in case study questions.

Case Study 1
42 Year old male . He comes into your office reporting a frequently recurring cough. The cough is particularly marked on rising in the morning with expectoration of abundant dilute white phlegm. He has accompanied signs of oppression in the chest, epigastric  fullness, nausea, poor appetite, and loose stools. 
Tongue: White slimy coating 
Pulse: Soggy or slippery
 
What is you TCM Diagnosis?
a. Invasion of lung by Wind dryness
b. Ke Sou (cough) due to Phlegm Dampness
c. Ke Sou (cough) due to Liver Fire
d. Ke Sou (cough) due Lung Yin Vacuity
